Registered Nurse (RN) Job Overview:
As a Registered Nurse, you will plan, organize, and deliver home care services, focusing on community health and patient education. You will work closely with individuals and families, ensuring personalized care in home and community settings.
Key Responsibilities:
- Conduct patient assessments, develop and adjust care plans, and administer treatments as prescribed.
- Provide direct nursing care, including medication administration, health education, and family counseling.
- Manage patient discharge planning and coordinate care with physicians and community health professionals.
- Supervise and evaluate home health aides’ care.
- Participate in on-call duties as needed.
- Graduate of an accredited nursing program with 1-2 years of recent acute care experience.
- Active state licensure and CPR certification.
- Preferred: Bachelor’s degree and home health care experience.
- Strong communication, problem-solving, and nursing skills.
