Job Summary:
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to join our team. In this role, you will provide direct patient care under the supervision of a registered nurse, ensuring high-quality, patient-centered services in a home care setting.
Key Responsibilities:
- Deliver direct patient care in compliance with state Nurse Practice Act guidelines.
- Implement care plans developed by the registered nurse.
- Accurately document patient progress and care provided.
- Educate patients and families on self-care techniques and treatment plans.
- Coordinate with other healthcare professionals, including HHAs, OTs, PTs, MSWs, STs, and Dietitians.
- Maintain and use medical equipment and supplies effectively.
- Participate in ongoing professional development.
- Perform additional duties as assigned.
- Graduate of an accredited LPN/LVN program.
- Minimum of 1 year of nursing experience (home health or medical/surgical preferred).
- Active LPN/LVN license in the state of practice.
- Valid CPR certification.
- Strong communication, documentation, and organizational skills.
- Valid driver’s license and insured, reliable transportation.
