About Us: Autris Healthcare is a growing home care provider dedicated to delivering compassionate, personalized care that enhances the quality of life for our clients. Job Summary: We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to join our team. In this role, you will provide direct patient care under the supervision of a registered nurse, ensuring high-quality, patient-centered services in a home care setting. Key Responsibilities:

Deliver direct patient care in compliance with state Nurse Practice Act guidelines.
Implement care plans developed by the registered nurse.
Accurately document patient progress and care provided.
Educate patients and families on self-care techniques and treatment plans.
Coordinate with other healthcare professionals, including HHAs, OTs, PTs, MSWs, STs, and Dietitians.
Maintain and use medical equipment and supplies effectively.
Participate in ongoing professional development.
Perform additional duties as assigned.

Qualifications:

Graduate of an accredited LPN/LVN program.
Minimum of 1 year of nursing experience (home health or medical/surgical preferred).
Active LPN/LVN license in the state of practice.
Valid CPR certification.
Strong communication, documentation, and organizational skills.
Valid driver’s license and insured, reliable transportation.
