Autris Healthcare is a growing home care provider dedicated to enhancing clients' quality of life through compassionate, personalized care. We are seeking a dedicated and experienced Physical Therapist to join our team and support our managed care initiatives.
Job Summary:
The Physical Therapist provides therapy services in accordance with a physician’s plan of care and state regulations. Responsibilities include assessing patient needs, developing treatment plans, guiding therapy sessions, and educating patients and caregivers.
Key Responsibilities:
Evaluate patients' therapeutic and rehabilitative needs.
Creates and executes treatment plans based on a physician's referral and patient assessment.
Instruct patients and caregivers on therapy techniques and equipment use.
Coordinate with physicians and other healthcare providers.
Monitor patient progress and adjust care plans as needed.
Supervise physical therapy assistants.
Qualifications:
Bachelor's or Master’s degree in Physical Therapy.
State licensure to practice as a Physical Therapist.
Minimum two years of experience (home health preferred).
Strong communication and organizational skills.
Current CPR certification.
Valid driver’s license and insured, reliable transportation.
