Job Summary:
The Physical Therapist provides therapy services in accordance with a physician’s plan of care and state regulations. Responsibilities include assessing patient needs, developing treatment plans, guiding therapy sessions, and educating patients and caregivers.
Key Responsibilities:
- Evaluate patients' therapeutic and rehabilitative needs.
- Creates and executes treatment plans based on a physician's referral and patient assessment.
- Instruct patients and caregivers on therapy techniques and equipment use.
- Coordinate with physicians and other healthcare providers.
- Monitor patient progress and adjust care plans as needed.
- Supervise physical therapy assistants.
- Bachelor's or Master’s degree in Physical Therapy.
- State licensure to practice as a Physical Therapist.
- Minimum two years of experience (home health preferred).
- Strong communication and organizational skills.
- Current CPR certification.
- Valid driver’s license and insured, reliable transportation.
